Psychiatry billing & RCM

Psychiatric billing sits at an awkward intersection: it uses medical evaluation and management coding, it layers psychotherapy on top of that, and it is frequently adjudicated by a behavioral health administrator operating under different rules than the medical plan. Each of those three facts creates its own denials.

Where psychiatric claims get complicated

The following describes general characteristics of psychiatric revenue cycles in the United States. Specific payer requirements vary and are confirmed per plan.

01

Evaluation and management with psychotherapy

A visit that includes both medical management and psychotherapy is billed as an E/M service with a psychotherapy add-on. The add-on is time-based and separate from the time supporting the E/M level, and the documentation has to make that separation visible. Where it doesn't, payers reduce or deny the add-on.

02

Diagnostic evaluation

Initial psychiatric diagnostic evaluations — with and without medical services — carry frequency expectations and payer-specific rules about repeat evaluations for the same patient. Re-evaluation billed too soon, or after a transfer of care without supporting rationale, is a recurring denial category.

03

Time-based documentation

Psychotherapy codes are defined by time ranges. The record must support the time actually spent in the psychotherapeutic service, distinct from time spent on medical management. Notes that reference a session generically, without the elements payers look for, are hard to defend on audit or appeal.

04

Medication management

Visits centred on prescription monitoring are E/M services and are levelled on medical decision-making or total time under current E/M rules. Under-levelling costs revenue; over-levelling creates audit exposure. Alignment between the documented complexity and the level billed is what protects both.

05

Telepsychiatry

A large proportion of psychiatric care is delivered remotely. Place-of-service codes, telehealth modifiers and payer policy differ between commercial plans, Medicare and Medicaid, and they have changed repeatedly. Claims that were correct last year are not automatically correct now.

06

Collaborative and integrated care

Practices delivering psychiatric collaborative care management or behavioral health integration services bill monthly, time-tracked codes with specific care-team and registry requirements. These are commonly under-billed simply because the tracking obligation isn't set up, and commonly denied when it isn't documented.

Common psychiatry denial drivers

  • Psychotherapy add-on billed without documentation separating it from E/M time
  • Claim routed to the medical plan rather than the behavioral health administrator
  • Telehealth place-of-service or modifier inconsistent with current payer policy
  • Authorization absent for a plan that requires it for outpatient psychiatric services
  • Diagnosis coding that doesn't support the service or the frequency billed
  • Provider not credentialed with that specific plan or product line
  • Repeat diagnostic evaluation billed inside the payer's frequency window

What we do for psychiatric practices

  • Verification that identifies the correct adjudicating entity and any authorization requirement
  • Coordination with your providers on documentation that supports the codes billed
  • Claim preparation aligned to each payer's current telehealth rules
  • Tracking of authorization units and review dates where the plan requires them
  • Denial triage that distinguishes coding issues from benefit and credentialing issues
  • A/R follow-up across both medical plans and behavioral health administrators
  • Reporting that shows where psychiatric revenue is being lost, and at which stage
Coding decisions remain the responsibility of the practice and its providers. We coordinate on billing accuracy and payer requirements; we do not substitute for clinical judgement or for a certified coding review where one is warranted.
